Fill out the appropriate boxes and submit for your payroll. Your paycheck will be delivered the following Friday after the end date of the pay period. You must submit this form the day after the last date of the pay period.

How many total administrative hours did you work during the pay period (for Admin only)?
Please list the dates and times for each day that you worked your administrative hours. This should match up with the number above. (for admin only)
For example, 3/8/2011, 3pm - 5pm
How many assessments did you perform during the pay period?
Please list the names of the assessments, the dates performed, and the times of the assessment.
For example, John Smith, 3/8/2011, 4:00pm
Please give the total number of training sessions worked during the pay period.
Please list any classes that you taught, meetings attended, days of prep work done, marketing research, and clinics performed. For the nutrition coach, dates and times for nutrition consults or seminars performed. Anything else please list here and describe. List the day and time of the event. Then at the bottom, what the number adds up to be for each event since different events might have different payrates.
